If you sign the ‘Consent to Accept Medicine’, we will understand that you have accepted the medicine correctly. When you and your doctor sign the ‘Consent to Administer Medicine’, we will understand that the medicine has been properly administered to you. After signing both the documents must be returned to our representative.

These two documents 'Certificate of Acceptance of Medicine' and 'Certificate of Administration of Medicine' are very important for us. If you fail to provide these documents to our representative, we will not be able to provide you with any free of cost medicine afterward.
